The Member’s Support Coordinator

In the Employment and Community First (ECF) CHOICES program, the member will have a Support Coordinator.

It’s important to know:

  • Who the Support Coordinator is

  • How to contact them

Email can be helpful so you have a record and can remember what you talked about and when you asked for things

The Support Coordinator helps the member:

  • Get health care, mental health care, and other services

  • Live in the community

  • Work toward their goals

What the Member’s Support Coordinator Should Do

  • Give clear information about Employment and Community First (ECF) CHOICES and answer any questions.

  • Help the member get the right services in the right place to meet their needs.

  • Help manage all of the members’ care—including medical care, mental health care, and long-term support needs.

  • Help solve problems and answer questions about the member’s care.

  • Check at least once a year to make sure the member still needs this level of care (like care in a nursing home), or is still “at risk” of needing it (for Group 5).

  • Talk with the member’s doctors and care providers to help everyone stay informed and work together.

  • Do a full checkup of the member’s health and care needs.

  • Help create a Person-Centered Support Plan—a plan that’s made just for the member, based on their goals and what they need.

Changing the Member’s Support Coordinator

If you’re not happy with the member’s Support Coordinator, you can ask for a different one:

When you call, explain why you are requesting a new Support Coordinator. If a new coordinator is available, they will assist with the change. If not, you can have your concerns documented with the current coordinator.

A Circle of Support is a team of people who come together to help the member plan for their life and reach their goals. This is called a Circle of Support (COS) meeting. The member is always the most important part of this team.

The Circle of Support Meeting includes:

  • The member

  • The member’s family or legal guardian/conservator

  • The Support Coordinator

  • The people and agencies that give the member services or supports

The member can also invite:

  • Friends

  • Advocates

  • Other people they trust, even if they aren’t paid to help

This team meets at least once a year to review and update the member’s Person-Centered Support Plan (PCSP).

You can have the right to request a Circle of Support meeting anytime by calling the member’s Support Coordinator.
