What is the Person-Centered Support Plan (PCSP)

Every member in ECF CHOICES must have a Person-Centered Support Plan, or support plan for short. This plan helps decide what services and supports the member will get. You should ALWAYS have a copy of this plan.

This plan is all about the member. It should be made in a way that makes sure:

  • The member leads the planning as much as they are able.

  • The member gets help to lead the planning if they need it.

  • The member has the information they need to make good choices.

  • Family, friends, or others the member chooses can help make the plan.

  • The member can speak for themselves or choose someone to speak for them.

  • If the member speaks or understands a different language, or communicates via American Sign Language an interpreter must be provided.

The planning process should also make sure:

  • The member can talk to their Support Coordinator before the planning meeting if they want to.

  • The member gets to pick who to invite to the meeting — and can say if they don’t want someone there.

  • The meeting is held at a time and place that works best for the member.

  • The member helps choose providers before services start — and can make changes at any time.

The support plan determines what services the member can get — and why TennCare should pay for those services.

The plan must show why the member needs the service and how it will help them reach their goals. The plan must also be updated annually (every year).

Example: If the member needs a computer to start a photography business, the plan must include a goal about getting a job or starting a business. It should also mention their interest in photography. If it doesn’t, the computer may not be approved.

Make sure the support plan clearly shows what the member wants and needs, so the right services can be approved or advocated for if denied.
